Responsibilities

• Provide comprehensive nutritional care to patients identified with special needs or nutritional risk
• Conduct patient assessment and reassessment to inform intervention planning
• Develop, implement, and monitor individualized nutrition care plans
• Educate patients and families about nutrition therapy and treatment goals
• Document all interventions in the medical record in accordance with policy and standards
• Collaborate with the interdisciplinary team to optimize patient outcomes and ensure care continuity

Requirements

• Ability to perform patient assessments and develop nutrition plans of care
• Strong documentation skills and precise charting in medical records
• Effective communication with patients, families, and health team members
• Competence in clinical nutrition care and decision-making processes
• Availability to work a daytime, 40-hour weekly schedule on a travel assignment
• Demonstrated ability to function effectively in a clinical setting
