Job Requirements

• American Heart Association BLS certification required for all direct patient care personnel
• State license or certification verification must be dated within 30 days of start
• Fit Test for masks required (specific mask brands and models listed)
• Trinity Code of Conduct Form must be signed
• Confidentiality and System Security Agreement must be signed
• Integrity and Audit Services Form must be signed within days prior to start
• Motor Vehicle Report must be dated within 30 days of start
• Color Vision test required
• MMR vaccination proof or IgG titer required
• Hepatitis B vaccination series required prior to start
• 2 Step PPD or IGRA TB test required with specific timing and documentation
• Varicella vaccination proof or IgG titer required
• Physical/Health Statement from exam within last year required
• Resume, Skills Checklist within a year, and 2 Professional References required
• Conglict Of Interest Disclosure Form must be signed
• Criminal Background Check within last 7 years required, dated within 30 days of start
• Educational Verification required
• EPLS report dated within past 30 days required
• GSA/OIG exclusion check dated within past 30 days required
• Joint Commission CORE Exam within a year required
• NSO report dated within 30 days required
• System Access Form must be completed one week before start
• Skills Checklist provided by agency required for clinical contractors
• Tdap vaccine or declination accepted if no pediatric interaction
• I9 form completed by agency required
• Covid-19 vaccine proof or declination/exemption required (with exceptions for CA and IL)
• 10 Panel Drug Screen completed within 30 days of start required
