Travel Physical Therapist Home Health Job

Start date: 09/21/2026 for a 13-week travel assignment in a hospital setting in Sonora, California.

Responsibilities

• Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions, while meeting department objectives
• Serve as co-leader in an interdisciplinary approach to patient groups
• Perform patient evaluations, develop and implement treatment plans, monitor progress, and adjust as needed
• Document patient progress and communicate with the care team; ensure compliance with documentation standards
• Educate patients and families about home exercise programs and safety

Requirements

• Active Physical Therapist license in California or eligibility to obtain licensure
• Graduation from an accredited physical therapy program
• Ability to work in a hospital setting as part of a travel assignment
• Strong collaboration and communication skills to work within an interdisciplinary team

Pay

Weekly pay: $2,593–$2,797 per week

Schedule

Days, 8-hour shifts (40 hours per week)
