Travel Physical Therapist - Home Health

Responsibilities

• Conduct home health visits including Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and routine visits.
• Maintain documentation using Homecare Homebase (HCHB).
• Serve patients with diagnoses such as CHF, orthopedic, and pneumonia across assigned areas.
• Ensure compliance with schedule, including rotating weekend shifts and holiday rotation.
• Provide quality patient care and collaborate with clinical support team.

Qualifications & Skills

• Minimum 1 year of physical therapy experience, home health experience required.
• Proficiency with HCHB, active state or NLC license, and BLS certification are mandatory.
• Valid driver's license and two professional references needed.
• Ability to perform in a home health setting, handle evaluations, and documentation efficiently.

Key Duties

• Manage patient care in a home setting, perform clinical evaluations, and ensure accurate charting.
• Collaborate with team for emergencies and support.
• Complete orientation and adhere to schedule and dress code.
• Utilize provided phone and tablet for work.
