We are seeking a Travel Home Health Physical Therapist to provide patient care in a home health setting.

Responsibilities

• Assessing and developing individualized treatment plans for patients.

• Monitoring patient progress and adjusting treatments accordingly.
• Collaborating with healthcare teams to ensure comprehensive care.

Qualifications

• Valid Physical Therapist license.
• Experience in home health or outpatient therapy preferred.
• Strong clinical assessment and treatment skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage a caseload effectively.
