Responsibilities
- Provide physical therapy services in a home care setting across multiple counties, including patient assessments, treatment planning, and delivery.
- Manage daily visits, document care using charting systems, and participate in on-call rotations.
Qualifications
- Valid KY PT license
- At least one year of acute care PT experience
- Strong communication, clinical judgment, and time management skills
- Home care experience and a valid driver's license with auto insurance are preferred
Key Duties
- Deliver therapy services to patients in their homes
- Travel between visits
