Travel Physical Therapist - Home Health - $3,100 per week

About the role

Travel Home Health Physical Therapist position in White River Junction, Vermont, serving over 70 towns across VT and NH. Full‑time day shift schedule (5×8‑hour days, 40 hours/week). Work in a home health setting as part of an interdisciplinary care team while maintaining independence in patient visits, with average visit length of 45 minutes.

Responsibilities

• Screen, evaluate, re‑evaluate, and treat patients
• Develop and carry out treatment plans
• Document evaluations and goals
• Teach patients and families about care rationale
• Communicate and coordinate treatment activities with the interdisciplinary care team

Requirements

• BS in Physical Therapy
• Current BLS certification
• Minimum 1 year of experience as a PT in a home healthcare setting
• PT license in Vermont and New Hampshire, or an unencumbered compact license (license required if applicable)
• Preferred: ASHA, CHT, or ACP training certifications
