Travel Physical Therapist Home Health - $2,540 per week

Travel Physical Therapist (Home Health) position in Gearhart, Oregon, serving Seaside, OR and surrounding areas. Start date: November 16, 2026. Duration: 13 weeks. Employment type: Travel. Responsibilities

• Evaluate patients and develop individualized care plans.
• Deliver skilled physical therapy services in patients’ homes.

• Maintain timely communication and documentation.

Requirements

• BCLS certification required.
• Active driver’s license and personal transportation essential.
• Minimum of 1 year of relevant home health experience.
• Experience with HCHB (Homecare Homebase) and Oasis systems required.

Qualifications

• Valid physical therapy license in hand.
• At least 1 year of skilled home health experience.
• Experience with home health OASIS documentation.
• Proficiency in HCHB system.
• Timely communication and documentation.
• Active driver’s license and personal transportation.
• Requested Time Off (RTO) must be submitted at application.
• Monday orientation required.

Pay

Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation).
