About the role
Medlivo Therapy is seeking a travel Home Health Physical Therapist for a 13-week contract starting July 27, 2026, in Lexington, Kentucky.
Responsibilities
- Provide physical therapy services in patients' homes
- Develop and implement individualized treatment plans
- Evaluate patient progress and adjust treatment plans accordingly
- Document patient care and treatment sessions accurately
Requirements
- 1 year of recent experience
- Active Physical Therapist License
- Current BLS/BCLS Certification
- Bachelor’s or Master’s Degree in Physical Therapy from an accredited PT program
