• Deliver direct intervention services, document progress, and adjust plans based on patient response
• Collaborate with interdisciplinary team members to support discharge planning, safety, and quality of life
• Maintain accurate documentation in appropriate records and ensure compliance with regulatory and facility standards
• Communicate clearly with residents, families, and team members to support goals and care continuity

Requirements

• Active Occupational Therapist license (state-specific) eligible for CT assignments
