Responsibilities

  • Conduct comprehensive OT assessments for adult and mixed-diagnosis patients in an acute care setting;
  • Develop and adjust individualized treatment plans aimed at improving functional mobility and activities of daily living;
  • Implement evidence-based OT interventions to address gait, balance, strength, dexterity, cognition, and ADL independence;
  • Monitor progress and adjust goals accordingly;
  • Collaborate with the multidisciplinary team (nurses, physicians, PT, speech-language pathology, social work) to coordinate discharge planning and ensure smooth transitions to the next level of care or home;
  • Educate patients and families on home exercise programs, adaptive equipment, safety strategies, and resources to facilitate safe community reintegration post-discharge;
  • Document evaluation findings, treatment plans, progress notes, and discharge summaries per program policy in the electronic health record;
  • Participate in rounding and care planning meetings; communicate any barriers to progress and advocate for patient needs within the acute care workflow;

Requirements

  • Active Hawaii OT license and certification to practice in Hawaii;
  • Ability to work in an acute care environment and adhere to hospital-level infection control and safety standards;
  • Availability to cover weekends and dayshift shifts as required by the assignment;
  • Proven ability to collaborate within a multidisciplinary team and document clearly in electronic health records;
  • Flexibility to work within a travel assignment framework and adapt to varying patient populations and acuity;
