Responsibilities
- Conduct comprehensive OT evaluations to determine functional status and goals for residents in a skilled care setting
- Develop and implement individualized treatment plans to improve activities of daily living and functional independence
- Facilitate therapeutic activities and progressive interventions aligned with short- and long-term goals
- Monitor progress, adjust plans as needed, and document outcomes in timely, accurate records
- Collaborate with an interdisciplinary team (nursing, therapy, social work) to coordinate care and discharge planning
Requirements
- Active Occupational Therapist license in California
- Experience in acute care or skilled nursing facility settings preferred
- Strong assessment, treatment planning, and documentation skills
- Able to work effectively within an interdisciplinary team
- Availability for a 2-week contract
