Responsibilities

• Educate patients on treatment plans, follow-up instructions, and self-management strategies.
• Perform medication reconciliation and communicate discrepancies.

Requirements

• Minimum of 2+ years of relevant experience.
• Experience in home health nursing with at least 2 years of practice.

Schedule

• 8-hour shifts.
