We are seeking an experienced RN for a travel home-health position.

Responsibilities

• Educate patients on treatment plans, follow-up care, and self-management
• Perform medication reconciliation and communicate discrepancies
• Document care, patient responses, and communications accurately
• Provide clinical updates to healthcare providers for timely decisions

Requirements

• At least 2+ years of clinical experience

Schedule

8-hour shifts, totaling 40 hours weekly
