About the Role
We are seeking an experienced RN in Case Management with a minimum of 2 years of clinical experience.
Responsibilities
- Documenting care, patient responses, and communications in medical records.
- Administering therapies and monitoring for adverse reactions or changes in patient condition.
- Performing medication reconciliation and communicating discrepancies.
- Supporting care coordination across healthcare providers to ensure continuity of care.
Skills and Qualifications
- Strong documentation and communication skills.
- Ability to manage patient care and therapies effectively.
- Experience in coordinating care among multiple providers.
Schedule
Work 40 hours per week with 8-hour shifts. Contract duration is 13 weeks.
