About the role
We are seeking an experienced RN - Case Management for a travel role in Garden Grove, California.
Responsibilities
- Document care provided, patient responses, and communications in medical records.
- Coordinate care across multiple providers to ensure continuity of care.
- Perform patient intake, focused assessments, and symptom screenings per clinic protocols.
Qualifications
- Minimum of 2+ years of nursing experience in case management or related fields.
Schedule
- Work 40 hours per week with 8-hour shifts.
