Responsibilities
Administer prescribed medications and monitor patient responses within a long-term care setting.
Perform routine nursing assessments, document findings, and communicate changes to the supervising RN/physician.
Provide basic bedside care, including assistance with activities of daily living, vital signs, and wound care.
Support care planning and coordinate with residents, families, and interdisciplinary team members.
Ensure accurate charting, medication administration records, and regulatory compliance.
Requirements
- Active LPN license in Vermont
- Ability to work in a long-term care environment
- Willingness to travel and work a flexible shift on a travel assignment
- Strong communication and teamwork skills
- Must be available to start on the specified date and commit to the assignment duration
