Responsibilities

  • Provide direct nursing care to residents in the long-term care setting in accordance with the plan of care, including routine assessments and treatment administration.
  • Administer medications and monitor responses, document in the MAR, and ensure safe medication handling practices.
  • Monitor patient status, take and interpret vital signs, assess pain, and report changes promptly to the supervising clinician.
  • Assist with activities of daily living and mobility, ensuring resident safety, comfort, and dignity.
  • Collaborate with an interdisciplinary team to update care plans, participate in rounds, and communicate changes to residents and families as appropriate.
  • Maintain accurate documentation, adhere to infection control protocols, and comply with state regulations and facility policies.

Requirements

  • Active Vermont Licensed Practical Nurse (LPN) license.
  • Current CPR/BLS certification for healthcare providers.
  • Practical Nursing diploma or state-approved equivalent.
  • Experience in a long-term care or skilled nursing setting preferred.
  • Ability to work flexible shifts and participate in a travel assignment.
  • Strong communication and teamwork skills; reliable documentation practices.
