Responsibilities

• Administer prescribed medications and treatments per physician orders and facility policy
• Maintain resident status and vital signs; escalate changes to the supervising RN
• Absorb activities of daily living, mobility, and personal care
• Perform wound care, dressing changes, and routine skin assessments
• Document care accurately in patient records and maintain care plans
• Collaborate with RNs, physicians, and the interdisciplinary team to coordinate ongoing care and safety

Requirements

• Active LPN license in Arizona
• Education: LPN diploma or equivalent
• Minimum 1 year of nursing experience; long-term care experience preferred
• Ability to work the night shift (3x12)
• Strong communication and teamwork skills; ability to work in a fast-paced setting
