Responsibilities
Administer medications and document administration per facility policy
Monitor residents’ conditions, perform routine vital signs, and report changes to the supervising RN
Assist with activities of daily living (ADLs) such as bathing, grooming, toileting, and mobility
Provide basic wound care and dressing changes as delegated
Maintain accurate and timely documentation in the charting system; ensure compliance with charting standards
Collaborate with RNs, CNAs, and other staff to coordinate patient care and ensure safety and quality of care
Requirements
Active LPN license in Pennsylvania
Ability to work night shifts in a long-term care setting
Prior experience in long-term care or geriatrics preferred
Strong communication and documentation skills
Ability to pass standard onboarding requirements (background check, drug screen) as applicable to contract
