Travel Licensed Practical Nurse Long Term Care Job

Responsibilities
- Observe residents continuously, identify changes in mental and physical well-being, communicate with the Supervisor, and provide detailed handoffs to on-coming shifts.
- Receive, transcribe, and implement physician orders; administer medications and treatments and document via MARs/TARs and electronic systems; manage unit pharmaceutical supplies.
- Supervise Licensed Nursing Assistants within NH Board of Nursing scope; assign tasks, monitor care quality, and assist staff as needed.
- Accompany physicians on rounds on assigned units to address medical needs and facilitate 60-day reviews.
- Administer oxygen and feedings per policy; perform wound care and manage wound vacs; monitor infection control practices and unit cleanliness.
- Manage unit operations including appointments, therapy schedules, nutrition charts, ambulation lists, and vital signs; maintain accurate documentation and report issues through the chain of command.
- Admit, discharge, and transfer residents; complete required documentation and notifications; prepare 24-hour shift reports.
- Communicate with residents’ families regarding orders and consent; participate in mandatory and non-mandatory meetings as assigned.
Requirements
- Active New Hampshire Licensed Practical Nurse license
- BLS (AHA) certification
- 3+ years of current clinical experience within the last 6 months
- Willingness to float to units with experienced staff
- IV-certified or willing to obtain during onboarding
- Covid-19 vaccination required (declinations/exemptions not accepted)
- RTO requests must be listed on initial submission and cannot exceed 5 days (holidays not approved)
- Locals residing within 50 miles of the facility will not be accepted; ensure perm address is in profile
- Previous travel experience highly preferred
- Rural/small facility experience highly preferred
- Resume, skills checklist (within the last year), 2 supervisory references from the last 2 years
- Active certifications and license required at submission
