Requirements

• Minimum 1 year of Skilled Home Health PT experience.
• Current BCLS certification.
• OASIS experience required.
• Pending Oregon PT license accepted.
• Reliable personal transportation required.
• Covered under COVID-19 primary vaccination series plus one booster.
