Travel Home Health Physical Therapist - $2,405 per week

Schedule

Days shift, 8 hours per day, with a minimum of 40 hours per week

Required Qualifications

• Valid license in hand
• Minimum of 1 year of relevant experience
• Proficiency in Homecare Homebase (HCHB)
• OASIS experience required for staff completing Start of Care (SOC)
• Timely communication and documentation
• Active drivers license
• Personal transportation

Certifications

BLS (Basic Life Support) required

Additional Requirements

• Must submit requested time off at the application
• Participate in on-call rotation if required
• Adhere to guidelines for hospice and home health care
• Complete orientation on the scheduled Monday

Pay

Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
