Travel Home Health Physical Therapist - $2,500 per week

SkyBridge Healthcare Therapy is seeking a travel Home Health Physical Therapist for a 13-week contract in Mount Vernon, Illinois.

Responsibilities

• Provide physical therapy services to patients in their homes.
• Develop and implement individualized treatment plans based on patient needs.
• Evaluate patient progress and adjust treatment plans accordingly.
• Document patient care activities and outcomes accurately.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Requirements

• At least 1 year full-time Physical Therapy experience in Home Health within the last 2 years.

Qualifications

• Current license to practice Physical Therapy in the state of Illinois.
• Passing score on the National Physical Therapy Examination (NPTE).
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Skills

• Strong clinical judgment and problem-solving abilities.

Pay

• Pay rate range: $49.50 - $58.50 per hour.
• Pay rate is dependent on seniority and other factors that will be discussed during the hiring process.
• Pay package includes a tax-free stipend.

Schedule

• 36 hours per week.
• 12-hour shifts.
