About the role
SkyBridge Healthcare Therapy is seeking a Travel Home Health Physical Therapist for a 13-week contract in Fort Wayne, Indiana.
Responsibilities
- Provide physical therapy services to patients in their homes.
- Develop and implement individualized treatment plans based on patient needs.
- Evaluate patient progress and adjust treatment plans as necessary.
- Document patient care activities and outcomes accurately.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Requirements
- At least 1 year full-time Physical Therapy experience in Home Health within the last 2 years.
Qualifications
- Current licensure to practice as a Physical Therapist in the state of Indiana.
- Passing score on the National Physical Therapy Examination (NPTE).
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.

- Pay rate is dependent on seniority and other factors that will be discussed during the hiring process.
