Responsibilities
- Provide one-on-one physical therapy in patients' homes
- Manage a daily patient schedule independently
- Complete OASIS documentation using Home Care Home Base
Requirements
- Active North Carolina PT license
- Minimum 1 year of Home Health experience
- OASIS experience required
- Home Care Home Base experience required
- Current BLS certification
- Valid driver's license and reliable transportation
