Ardor Health Solutions is seeking an experienced Home Health Physical Therapist for a 13-week assignment in Camden, South Carolina. This opportunity is ideal for a PT who enjoys autonomy, flexible scheduling, and providing one-on-one patient care in the home setting.

Client: BrightStar / Amedisys Home Health
Location: Camden, SC
Contract Length: 13 weeks
Schedule: Monday–Friday, Day Shift (8 hours per day, 40 hours per week)

Responsibilities
- Provide one-on-one patient care in the home setting
- Manage a daily patient schedule independently
- Complete OASIS documentation
- Utilize Home Care Home Base for clinical documentation
Requirements
- Active South Carolina PT License
- Minimum 1 year of Home Health experience
- OASIS experience required
- Home Care Home Base experience required
- Current BLS Certification
- Valid driver's license and reliable transportation
