We are seeking a skilled Home Health Occupational Therapist to provide high-quality patient care in the West Portland area. The ideal candidate will have a passion for enhancing the quality of life for patients in a home setting, utilizing their expertise to facilitate recovery and rehabilitation.
Responsibilities
- Conduct patient evaluations and develop individualized treatment plans.
- Implement therapeutic interventions to enhance patient independence and quality of life.
- Document patient progress and adjust care plans as necessary.
- Educate patients and families about treatment plans and goals.
- Collaborate with healthcare professionals to ensure comprehensive care.
- Utilize OASIS for patient documentation as required.
Requirements
- Must hold a valid Oregon Occupational Therapist license.
- Minimum of 1 year of skilled home health experience is required.
- BCLS certification is necessary.
- Experience with OASIS documentation is highly preferred.
- Reliable transportation is required for travel between patient visits.
