Travel Occupational Therapist - Home Health - $2,596 per week

About the role

We are seeking a licensed Occupational Therapist (OT) to join our Home Health team in West Portland, OR. The OT will provide valuable therapy services to patients in their homes, focusing on rehabilitation and skill development for improved daily living.

Responsibilities

• Conduct patient evaluations and develop individualized treatment plans.
• Implement therapeutic interventions to promote functional independence.
• Educate patients and families on home health care techniques and tools.
• Document patient progress and modify treatment plans as needed, utilizing OASIS standards.
• Aid patients with adaptive equipment and home modifications.

Requirements

• Current Occupational Therapist license in Oregon.
• Minimum of 1 year of skilled home health experience is required.
• BCLS certification is required.
• Experience with OASIS documentation is preferred.
• Reliable transportation for travel between patient locations.

Qualifications

• Must be able to pass a drug screen and background check.
• Must have a valid driver’s license and reliable transportation.

Pay & Schedule

The pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
