Travel Occupational Therapist - Home Health - $2,593 per week

About the Role

Ardor Health Solutions is seeking a Travel Occupational Therapist for a Home Health contract in White River Junction, Vermont. This opportunity offers the independence and flexibility of home health while providing meaningful one-on-one care to patients in their homes. Ideal for an OT who enjoys autonomy, patient-centered care, and helping individuals improve safety, independence, ADLs, and overall function in the home environment.

This is a full-time, 13-week travel assignment with a 36-hour weekly guarantee, day shift, and an adult & geriatric patient population.

Responsibilities

• Perform comprehensive occupational therapy evaluations and reassessments.
• Develop individualized treatment plans based on patient needs and physician orders.
• Treat patients focused on ADLs, safety, mobility, functional independence, and home management.
• Provide education on adaptive equipment, home modifications, safety strategies, and caregiver support.
• Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.
• Complete timely documentation and maintain home health compliance standards.
• Manage a home health caseload while coordinating visits based on patient needs.

Requirements

• Vermont and New Hampshire OT licensure required, or applicable compact license.
• BLS certification required.
• NBCOT certification required.
• Home Health experience preferred.
