About the role

We are seeking a travel Home Health Occupational Therapist for a 13-week assignment in Arbor Vitae, WI. This role offers the opportunity to make a significant impact by providing therapy in the home setting.

Responsibilities

• Assess patients' needs and develop personalized treatment plans in the home setting
• Provide therapy to improve daily living skills and overall functions
• Collaborate with caregivers and families to support patient goals
• Educate patients and families on adaptive strategies and home modifications
• Monitor and document patient progress and outcomes regularly
• Implement interventions tailored to the home environment
• Participate in care coordination with healthcare teams and community resources

Requirements

• Master's degree in Occupational Therapy
• WI Occupational Therapy Licensure
• Clean driving record

Pay

$2,081.97 - $2,146.36 per week

Schedule

• 13-week assignment
• Day shift, 8 hours per day, up to 40 hours per week
• On-site in a home health setting
