• Certifications: ACLS (Copy needed); BLS (Copy needed); PALS (Copy needed); CERT-TNCC (Copy needed)
• SSN Required
• DOB Required
• References: 1 Reference in entire work history
• License in hand
• 1 year of Level I or large Level II experience required
