Responsibilities
- Provide rehabilitation services as a COTA in a SNF/ICF setting, including evaluations, reassessments, treatments, and related clinical care within licensure, discipline, and scope of practice
- Develop, implement, or carry out individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary team relevant to the role
- Implement evidence-based therapeutic interventions to maximize function, independence, communication, mobility, swallowing, and quality of life as applicable
- Educate residents, families, and caregivers on treatment plans, progress, and discharge planning; document progress and changes in plan of care accurately and timely
- Supervise therapy assistants when applicable; ensure documentation complies with facility, state, federal, and payer requirements
- Maintain flexibility to work holidays, and participate in department coverage; may float to other units or facilities based on census and staffing needs
- Must arrive the Thursday before start to attend employee health appointment as required
Requirements
- Current Hawaii State license in good standing appropriate to the discipline
- Current Basic Life Support (BLS) certification
- Minimum two (2) years of SNF/ICF experience preferred
- Ability to practice within the scope of licensure and state regulations; capacity to work in a SNF/ICF setting
- If not currently licensed in Hawaii, must apply and obtain Hawaii license by start of assignment
About the Role
This assignment supports rehabilitation services in a skilled nursing/intermediate care facility, with responsibilities aligned to licensure, scope of practice, and state regulations. Hawaii state licensure must be active by start, and the traveler will receive onboarding for EMR systems and facility workflows.
