PACE Program
The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.
Responsibilities
- Schedule and adjust daily transportation routes
- Cook with participants, caregivers, healthcare providers, and drivers
- Ensure all transportation needs are met safely and efficiently
- Manage changes to appointments and transportation requests
- Maintain accurate records
- Communicate effectively with the interdisciplinary care team
Qualifications
- High school diploma or GED
- Strong organizational and time-management skills
- Excellent verbal and written communication skills
- Ability to multitask and prioritize in a fast-paced environment
- Proficiency with Microsoft Office applications and scheduling software
- Customer service experience and the ability to interact professionally with participants, caregivers, and staff
- Ability to maintain accurate records and attention to detail
- Ability to work independently and as part of a team
Preferred Qualifications
- Previous scheduling, dispatching, or transportation coordination experience
- Experience in a healthcare, senior services, or PACE environment
- Knowledge of medical appointment scheduling and healthcare operations
- Experience using electronic health records (EHR) systems
- Familiarity with transportation routing and logistics software
- Associate degree or additional education in healthcare administration, business, or a related field
- Experience working with older adults or individuals with complex healthcare needs
