About the Role

A Transplant Nurse Coordinator is responsible for providing and coordinating clinical care for transplant candidates and recipients.

Responsibilities

  • Conducts pre-transplant interviews and manages evaluations.
  • Communicates with physicians and facilitates patient placement on transplant lists.
  • Participates in transplant team goals and develops educational programs.
  • Conducts patient visits pre- and post-transplant.
  • Engages in research, interprets findings, and contributes to quality improvement initiatives.
  • Represents the organization at various levels through speaking and publishing.
  • Ensures age-specific patient care and models quality service.
  • Maintains confidentiality and mentors staff.

Requirements

  • Current Connecticut RN license.
  • BSN required.
  • 2-4 years recent acute care experience.
  • Transplant experience and specialty certification preferred.

  • Demonstrated knowledge of transplant care.
