RN Kidney Transplant Coordinator
Preferred nursing experience: Transplant, nephrology, ICU, and surgical.
Schedule: Full-Time, 40 hours/week. The initial schedule for this position will be Monday through Friday, 8:00 AM to 4:30 PM, during a 90-day orientation period. After orientation, there may be an opportunity to transition to a 4-day, 10-hour schedule, depending on the candidate and departmental needs. On-call responsibilities occur approximately 10 days per month and are handled remotely—no on-site presence is required during on-call hours.
Responsibilities
Post-Transplant/Donation Period
- Coordinate outpatient care to optimize recipient recovery and quality of life.
- Assess and support patient adherence to treatment and medication regimens.
- Identify complications and collaborate with providers to determine interventions.
- Arrange diagnostic testing, hospital admissions, and emergent evaluations.
- Ensure data collection and timely reporting to regulatory bodies.
- Demonstrate understanding of post-transplant/donation management including medications, complications, interventions, and discharge planning.
- Coordinate care of the transplant/living donor recipient in the outpatient setting to achieve optimal physical, social, and emotional rehabilitation.
- Coordinate/ensure discharge planning to provide for continuity of care.
- Assess recipient's compliance with medication and medical regimen and collaborate with the transplant team to attain and maintain compliance.
- Collect and review data pertinent to recipient/donor health (and organ function, if applicable) with the transplant physician.
- Communicate adjustments in patient plan of care and drug transplant therapy as ordered by medical providers.
- Arrange for consults, diagnostic testing, and hospitalization when indicated.
- Coordinate emergent evaluation of transplant recipient when indicated.
- Assist in reporting required data to regulatory bodies.
Perioperative Period
- Facilitate transplant/donation process in alignment with clinical standards.
- Share recipient/donor information with transplant team and regulatory agencies.
- Coordinate admissions and surgical scheduling, and manage patient listing status.
- Participate in transplant/donation process according to specific standards.
- Share pertinent information about the intended recipient, including ABO compatibility, with the transplant team.
- Facilitate admission or transfer of patient to the transplant facility.
- Notify transplant team of pending medical and/or surgical procedure.
- Remove transplant recipient/living donor from list according to regulatory guidelines.
- Provide outcome specifics to regulatory bodies for donor family follow-up.
Pre-Transplant/Donation Period
- Monitor and guide candidates through evaluation and pre-transplant readiness.
- Address psychosocial, educational, and financial needs during the pre-transplant phase.
- Collaborate on re-evaluation and maintain compliance with listing requirements.
- Monitor and coordinate care of the candidate awaiting transplantation/living donation.
- Ensure monitoring per transplant center-specific standards.
- Identify and respond to the psychosocial and economic needs of the transplant/living donor candidate during the pre-transplant/donation period.
- Exhibit ability to maintain and update candidate listing status per regulatory regulations.
- Identify the need for candidate re-evaluation and initiate the process when appropriate.
- Act as a resource for internal and external healthcare providers.
Referral and Evaluation
- Oversee and coordinate evaluation of transplant/living donor candidates.
- Analyze medical and psychosocial data, and communicate with referring providers.
- Support regulatory documentation and transplant candidate advocacy.
- Facilitate/coordinate evaluation and listing of potential transplant/living donor candidates.
- Identify requirements and provide oversight of the candidate evaluation process.
- Interpret serological results and their implications for transplant.
- Gather and communicate serological results and blood type in accordance with regulatory requirements.
- Identify possible comorbidities and absolute/relative contraindications for potential transplant recipients/living donor candidates.
- Function as candidate advocate by applying knowledge of growth, development, educational, and cultural background during candidate/family interactions.
- Collect and evaluate medical, psychosocial, and financial data for review by the transplant team.
- Communicate with referring physicians, other care providers, payors, and transplant team members regarding candidate status and evaluation outcome.
- Provide data to regulatory bodies for listing statistics.
Communication & Collaboration
- Maintain detailed electronic health records and patient documentation.
- Serve as a liaison between patients, families, providers, and external partners.
- Collaborate across disciplines to build and implement individualized care plans.
- Maintain thorough, accurate, and timely electronic documentation of all patient encounters to ensure safe transitions between all transplant/donation-specific phases of care.
- Communicate effectively with patients, family, and multidisciplinary team members, as well as referring and primary care providers and consultants regarding patient clinical findings and plan of care in a timely manner.
- Maintain/ensure communication with patient, referring physician, payors, or other healthcare providers throughout the transplant/donation process.
- Collaborate with nursing staff, social work, and other healthcare providers to establish and implement patient care plans according to current standards of care.
Requirements
- Bachelor’s degree in Nursing or a related field (required).
- ACEN or CCNE accredited BSN or MSN, OR Associate/Diploma RNs with 2 years of experience and a BS.
- Preferred: MSN.
- Active RN license in the state of Ohio (or ability to obtain).
- Minimum 5 years of clinical nursing experience, preferably in transplant, nephrology, or critical care.
- Excellent communication, organizational, and collaborative skills.
- Ability to manage complex clinical scenarios with compassion and precision.

Pay
Expected starting pay range: $81,723.20 - $104,208.00. Starting pay is based on experience, skills, and equity; exceptions may apply for highly qualified candidates. Additional pay (e.g., shift, on-call, or weekend differentials) and benefits may apply. Annual pay may vary based on FTE status.
