Schedule

Full-time days, Monday-Friday, five 8-hour days or four 10-hour days. Required "after hour on call" approximately three days a month.

About the Role

As a Transplant Coordinator RN, utilize a nursing model for quality patient care. Assess patient needs and create personalized care plans. Watch how patients respond to personalized care. Ensure patients have smooth transitions through healthcare. Prevent complications or delays by understanding needs.

Responsibilities

• Assessing transplant eligibility for patients involves interviewing them and their families.
• Coordinate tests for transplant candidates, identify issues in results, and arrange necessary care.
• Work closely with patients, physicians, and other healthcare providers during this time.
• Manage recipient's post-transplant care.
• Arrange admissions, lab tests, clinic visits.
• Coordinate with referring, primary care.
• Understand test results, identify irregularities, and set up needed care as directed.
• Teach candidates, families, and caregivers about evaluation, organ allocation, waiting, post-operative, immunosuppression effects, complications, and psychological impact of transplant.
• Use insurance knowledge to help with post-transplant prescriptions, assist patients with coverage issues, and maintain long-term prescriptions.
• Comply with various regulatory policies and maintain records on the transplant waitlist through different online programs.
• Coordinate recipient transplants includes using the donor website and communicating with potential recipients. Assess their health status and notify hospital departments. Updates are also sent to the team.

Requirements

• Associate's degree level.
• Two years of professional experience.
• An (RN) Registered Nurse License.
• Basic Life Support (BLS) skills required or obtain within 30 days if not already held.

Skills

• Comprehensive skills in nursing and patient care standards and procedures.
• Knowledge of rules, laws, regulations, standards, guidelines, certifying bodies, and accrediting bodies relevant to the department.
• Knowledge of medical terms, health practices, disease control, medication specifications, and side effects.
• Respect for, and diligent observance of, professional nursing ethics and patient privacy rights.
• Excellent communication skills, both spoken and written, for clear transmission of ideas.
• Basic computer skills: Microsoft Office, information security, scheduling systems, payroll systems, electronic medical records, and email use.
