RN - Kidney Transplant Coordinator
Job Summary
The RN Transplant will coordinate a focused and effective effort between the transplant center and major patient referral sources for renal transplantation. Responsibilities include:
- Assuring timely and efficient transplant education, work up, updates, and activation for transplantation.
- Managing a designated caseload of patients during the pre/post-recipient and pre/post-donor transplant process.
- Coordinating referrals and addressing roadblocks within the transplant workup process.
- Providing patient, family, and staff educational programming.
- Affirming that policies, procedures, and regulatory requirements are met to ensure compliance with governing agencies.
- Offering outreach services to regional dialysis and nephrology programs.
Qualifications
- Graduate from an accredited nursing program.
- Bachelor’s Degree in nursing required, or in the process of acquiring with an estimated completion date within five years.
- Currently holds an unencumbered RN license with the State Board of Nursing.
- Minnnesota RN license is required within six months of date into position.
- Obtains and maintains required department specific competencies and certifications.

Pay
$34.00 - $48.00 per hour. $10,000 sign-on bonus available for qualified candidates!
Schedule
Full time, 8 Hours - Day Shifts, 40.00 weekly hours.
Department Details
This Transplant Coordinator works with post-transplant patients providing education and coordination of care. The team has completed 954 Kidney Transplants since the start of the program in 1989.
