Sentara Healthcare is seeking a Full Time/Day Shift Transplant Coordinator for their Kidney/Pancreas Transplant team at Sentara Norfolk General Hospital. Sign on bonus available for qualified candidate!

What you will do

• Care coordination with post-transplant patients.
• Facilitates clinic flow, transplant evaluation, and patient care management.
• Provides weekly education sessions to patients and support members.
• Arranges appointments with surgeons, social worker, financial coordinator, pharmacy, and dietary.
• Admits patient, provides support during the waiting period, and discharges transplant patient with instructions for going home.
• Manages a case load of patients, remains patient’s point of contact throughout the transplant process, recovery, and at home.
• Requires a Registered Nurse, BSN with 2+ years of acute care experience, good medical - renal knowledge, and understanding of chronic disease. BLS required. Certified Clinical Transplant Coord (CCTC) required within three (3) years of hire.
• The outpatient clinic operates Monday – Friday between 8am and 4:30pm. Transplant Coordinator will have some scheduled call and have weekend call about every 11th week.
