Responsibilities
- Collaborate with DDS and the Regional Centers on the Intake and Referral Process.
- Facilitate and lead Intake/admission meetings.
- Collaborate with the Clinical Coordinator, Consultants, and other members of the IDT in the development and implementation of the IPP and Person-Centered Plan and transition plans.
- Assist in conducting Risk Assessments in collaboration with the Director and Clinical Director.
- Coordinate with Regional Center Liaison and Service Coordinator on accessing services in the community as per IPP and Transition Plan.
- Support the person pre-transition, during transition, and post-transition with a minimum of monthly visits or more frequently as necessary.
- Perform all other duties as assigned.
Requirements
- A Master’s Degree in Psychology, Social Work, or Counseling.
- Five years of experience in the past 7 years working with people with IDD with behavioral/mental health concerns with criminal and/or forensic involvement.
- Extensive experience in the development and implementation of Individual Program Plans and person-centered plans for people with IDD with criminal and forensic involvement.
- Extensive experience in linking people with IDD with complex behavioral/mental health and forensic needs to community supports and resources.
- Essential physical capabilities include lifting, assisting with lifting, and standing.
Preferred Qualifications
- Credentialed Alcoholism and Substance Abuse Counselor (CASAC) credential.
Pay
$87,560.30 annually
