Responsibilities

  • Following MassAbility policies, procedures and reporting in a timely manner
  • Aiding with housing search, voucher and housing applications
  • Attending housing appointments with participants for interviews, tours, and other sessions
  • Aiding with core documents applications required by housing authorities, such as an ID or Social Security Card
  • Setting up apartments according to participant preferences
  • Maintaining active communication with MassAbility case managers with updates on participants
  • Attending transition meetings prior to participant discharge
  • Tracking and documenting direct services, housing applications, shopping expenses, and all communications related to participants
  • Communicating with MassAbility CM and other members of the transition team prior to discharge
  • Coordinating with MassAbility CM on home modifications needs
  • Initiating home assessments on housing spaces for suitable housing goods and furnishings
  • Purchasing furniture, housing goods, and all essential items based on participant preferences
  • Scheduling utilities services as needed
  • Adequately updating service activity logs, delivery reports, and submitting reimbursements in a timely fashion
  • Attending interdisciplinary training, in-service, and meetings to maintain and improve TA service-related knowledge
  • To assist in the development of information concerning care plans
  • To arrange transportation under TA plan and/or transport individuals in their own vehicle

Requirements

  • A college degree (preferably in a human service field), plus experience in providing community-based services to individuals with disabilities; or at least 2 years comparable, community-based, life or work experience providing services to individuals with disabilities and/or persons 60 years or older
  • A valid driver's license, clean driving record, and a reliable, clean, insured vehicle that has had manufacturer’s recommended scheduled services and transports clients as appropriate
  • To work flexibly to meet the occasional requirements to work outside normal hours
  • To maintain confidentiality of sensitive information
  • To utilize effective time management techniques to ensure tasks are prioritized, and agreed deadlines are met
  • To attend all relevant supervision and meetings
  • To comply with State/Federal regulatory acts and agency policies
  • To respect and maintain client confidentiality at all times
  • To report concerns of client abuse or neglect
  • To refrain from discussing personal problems, concerns, or opinions with clients, or client representatives
  • To communicate effectively in the language and communication style of the Participants to whom they provide services and their families
  • To have written and oral communication skills, problem-solving skills, planning and budgeting skills, and math aptitude
  • To demonstrate professionalism
  • To lift and move up to 75 lbs.
