TRANSITION SERVICES COORDINATOR

About the role

Rite of Passage Team is hiring a Transition Services Coordinator at Les Peters Youth Academy in Tampa, Florida. The position is open to applicants from the St. Petersburg, FL area, as the site will be relocating.

Responsibilities

• Supervise a caseload of approximately 14-24 students.
• Assist with facilitating court hearings, Probation Officer visits, student visits, and/or home passes.
• Meet with students to review progress.
• Primary program liaison between student, parents/legal guardian, and placing agency officials.
• Provide monthly progress notes and weekly student updates.
• Cook up activities with other program staff.
• Participate in assessments and treatment plans.

Requirements

• A bachelor's degree or better.
• Pass a criminal background check.
• Pass a drug screening.
• Pass a physical exam.
• Pass a TB test.

Qualifications

To be considered, you should have:

• Experience working with youth.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Knowledge of mental health services and interventions.

• Strong written and verbal communication skills.
• Ability to build and maintain positive relationships with youth, families, and staff.
