About the role
This Therapist will be responsible for providing in-person, telehealth, and on-call Crisis support for Adult populations in Preble County. SBHI is the mobile adult crisis responder in Preble County during our hours of operation.
Responsibilities
- Provide for Crisis support including de-escalation, assessment, clinical support, and hospitalization (pink-slipping) clients who present as walk-ins or in the community.
- Monitor Crisis Notifications, connect patients with ongoing support and services, and provide outreach to patients.
- Accompany Case Managers into the field to assist with the assessment and support of clients with SPMI.
- Participate in and lead trainings for various stakeholders in the community around the topic of Crisis Intervention and De-escalation.
- Work collaboratively with the local Crisis Hotline, law enforcement, juvenile court, schools, or other community agencies to receive calls for MRSS Outreach to the above indicated entities to publicize program.
- Respond to calls for MRSS Outreach to the above indicated entities to publicize program (minimal contacts will be established).
- Triage referrals by calling the individual/family in crisis.
- Face-to-Face Mobile Response time of 20-60 minutes. These mobile locations may be at home, school, court, etc. This timeframe can be accomplished through positioning staff in satellite locations.
- Anticipated Face-to-Face response (vs. via phone only) will be at least 75% of all referrals/requests.
- Provide caregiving support up to 45-60 days.
- Data collection/reporting on: Number of calls/referrals by Day & time, Response time: via phone and/or face-to-face, Location of response, Outcome of response, Linkages achieved during Stabilization (how many and with what agency), Length of time involved in Stabilization services, Amount billed/collected from Medicaid and Private Insurance, Evaluation consent form completion by parent/guardian.
- Note: Further components may be added or changed as the project develops.
- Working with electronic medical records system and documenting patient interactions.
- For sustainability purposes, services, to the fullest extent possible, should be billed to Medicaid and/or private insurance.
- Other duties as assigned.
Requirements
- Licensed Professional Counselor (LPC), Licensed Social Worker (LSW), Licensed Marriage and Family Therapist (MFT)
- Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) or Licensed Marriage & Family Therapist (LMFT)
- Certified in CPR within 30 days of hire
- Valid Driver’s License and personal vehicle required
