Job Summary This role involves evaluating and assessing patients to establish measurable, functional goals aimed at restoring abilities. The practitioner provides treatment based on a written plan of care, monitors progress, and adjusts interventions as needed. Responsibilities include documenting care, communicating evaluation results, and developing discharge plans with patient and family education. The role may include supervising students and support staff, and contributing to healthcare improvement initiatives. Adherence to ethical standards and a caring approach are essential. Responsibilities Assess patients using subjective and objective data.Share evaluation results with patients and caregivers.Deliver treatment aligned with the plan of care.Document interventions in medical records.Reassess patient progress and modify plans accordingly.Provide discharge planning, education, and referrals.Support staff and student development.Participate in initiatives to improve healthcare delivery.Complete assigned goals effectively. Qualifications & Skills Graduate from an accredited Speech-Language Pathology program.Possess SLP-CCC licensure.Hold BLS certification within 14 days of hire.Basic computer literacy skills such as keyboarding and navigation.
