About the role
This position is an at-will direct service role within a program. The primary responsibility is to assist and advocate for members in all areas of treatment and help them access and utilize available services.
Responsibilities
- Works in partnership with the provider and the parents, recognizing them as experts on their families.
- Completes intake paperwork and conducts behavioral and strength assessments.
- Facilitates meetings with the child, family, lead provider, and other stakeholders.
- Collaborates on treatment plans with the lead provider.
- Assists family and lead provider in implementing the treatment plan.
- Implements performance outcome evaluations, working with the child and family to enlist the involvement of formal and informal community resources.
- Serves the family weekly, adjusting meeting times as needed based on family needs.
- Develops discharge planning and transitional services.
- Ensures all required records are kept up-to-date and accurate.
- Provides prompt intervention during crises, notifying relevant parties and providing on-the-spot counseling.
- Attends to the safety, health, and well-being of clients.
- Participates in periodic after-hours on-call duties on a rotational basis.
- Attends child/family team meetings at other lead provider agencies as needed.
- Committed to a wrap-around philosophy when working with families.
Requirements
- Graduation from an accredited college or university with a bachelor’s degree in Social Work, Psychology, or a related field.
- Four (4) years of varied experience counseling mentally disabled persons.
- California driver’s license and current vehicle insurance/registration if driving.
- Reliable means of transportation capable of passing vehicle safety inspection if more than five years old, excluding all modes of two-wheeled transport inclusive of bicycles, mopeds, and motorcycles.

- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Experience with community resource coordination.
- Basic understanding of legal and ethical standards in social work.
