Primary Duties And Responsibilities
- Coordinate and communicate with practice staff
- Research patient demographic and clinical data
- Make outreach calls to patients
- Auxiliary patient scheduling and other administrative tasks
- Document outcomes
- Support the overall mission of the quality team by assisting with Stars/quality projects
- Work with practices as needed to support their quality work
- Perform other administrative functions to support the quality team
Essential Functions

- Collaborates with the clinical quality team and practice staff to provide patient outreach
- Encourage medication adherence, explore barriers, provide education, and facilitate necessary steps
- Interact with primary care practices and pharmacies to coordinate follow-up actions
- Document patient interactions accurately and with great attention to detail
- Communicate accurately, effectively, and in a timely manner with all stakeholders
- Develop familiarity with clinical and medical terminology pertinent to the patient population
- Identify opportunities for collaboration and leverage ancillary community resources
- Resolve issues and respond to requests in a timely and efficient manner
- Provide education and support to PCP offices in developing effective and efficient processes
- Enter data into electronic medical records or quality platform as directed
- Perform administrative functions to support the quality team
- Use, protect, and disclose patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
- Perform additional duties, as assigned
Required Qualifications
- Minimum: High school diploma, G.E.D. or equivalent
- Preferred: Associate’s or Bachelor’s Degree
- Minimum: 3 years of professional experience in a physician practice, hospital, pharmacy, or health plan
- 3 years of professional experience in a customer-oriented setting
- Superior interpersonal skills; demonstrates the ability to engage and motivate patients, physicians, and PCP office staff telephonically
- Organized, detailed, accurate
- Self-motivated and proactive; responds to requests in a timely and efficient manner
- Demonstrates success in project accountability and customer responsiveness
- Demonstrate sound decision-making skills and demonstrate the ability to work independently with supportive activities and interactions with both internal and external customers
- Customer-service oriented; responsive and respectful to internal and external customers
- Excellent communication skills; ability to represent the organization with skill and confidence
- Collaborative and resourceful; functions successfully in a team environment as well as independently
- Computer literate with proficiency in Microsoft Office suite of programs
- Experience working in electronic medical records
- Proficient with web-based search engines and software applications, office hardware (phones, fax machines, etc.)
- Ability to type at least 45 wpm
- Knowledge and familiarity with medical and pharmacological terminology
- Knowledge and familiarity with Stars and HEDIS measures
- Working knowledge of HMO/managed care/health plans
