Responsibilities

  • Provide intensive case management services to individuals experiencing homelessness
  • Implement the Housing First model
  • Assist clients in accessing necessary resources such as housing, healthcare, and employment
  • Maintain regular contact with clients to monitor progress and address needs
  • Collaborate with other service providers and community organizations

Requirements

  • Bachelor’s degree in social work, psychology, or related field
  • At least 2 years of relevant experience in social services or related field
  • Valid driver’s license and access to reliable transportation
  • Ability to work flexible hours including evenings and weekends
  • Qualifications

    • Strong interpersonal and communication skills
    • Proficient in Microsoft Office Suite
    • Experience with data entry and record-keeping systems

    Skills

    • Excellent organizational and time management skills
    • Ability to work independently and as part of a team
    • Knowledge of local social service agencies and resources
