Role Overview
The National On Call advanced practice clinician (APC) provides telehealth care, guiding patients, caregivers, and facility staff 24/7, including holidays. This remote, part-time role involves delivering virtual acute, chronic, and behavioral health assessments, documenting findings per regulatory standards, and utilizing EMR systems efficiently.
Key Responsibilities
- Coordinate care through telephonic and video platforms, ensuring patient privacy.
- Manage acute and chronic conditions collaboratively with physicians and specialists.
- Conduct comprehensive assessments, document accurately, and ensure quality standards.
- Facilitate care transitions, monitor patient needs, and support care plan adjustments.
- Maintain excellent customer service and communication with all stakeholders.
Qualifications & Skills
- Master’s or Doctorate in Nursing with active, unrestricted NP license and national certification (FNP, ANP, GNP, AGACNP).
- DEA licensure or ability to obtain post-hire.
- Proficient in EMR documentation and virtual communication tools.
- Minimum of 3 years clinical experience as an APC preferred.
- Ability to work flexible shifts, including weekends and holidays.
