Telephonic Case Manager RN Behavioral Health - Remote

About the role

The Telephonic Case Manager RN Behavioral Health supports a diverse member population with education, advocacy, and connections to the resources needed for improved health outcomes. This role involves identifying gaps in treatment plans, providing patient education, making referrals, and maintaining continuous communication with members.

Responsibilities

• Make outbound calls and receive inbound calls to assess members' current health status
• Identify gaps or barriers in treatment plans
• Provide patient education to assist with self-management
• Make referrals to outside sources
• Support members with condition education, medication reviews, and connections to resources like Home Health Aides or Meals on Wheels
• Communicate effectively with members via in-person or on-phone interactions
• Work in a high-volume, customer service environment

Requirements

• Current, unrestricted RN license in the state of employment
• Active Compact RN License
• 1+ years of experience with mental health/behavioral health/substance use focus
• Proven ability to type and navigate a Windows-based environment
• Access to high-speed internet (DSL or Cable)
• Dedicated work area established that is separate from other living areas and provides information privacy

Preferred Qualifications

• BSN
• Certified Case Manager (CCM)
• 3+ years of experience with a mental health/behavioral health/substance use focus
• Case management experience
• Experience or exposure to discharge planning
• Experience in a telephonic role

Pay

The salary for this role ranges from $60,200 - $107,400 annually based on full-time employment. Employees must pass a drug test before starting the job.

Schedule

This is a full-time, Monday - Friday, 8am-5pm position in your time zone. You can work remotely from anywhere within the U.S.
