Remote RN Case Manager - Behavioral Health
About the role
This role blends clinical expertise with a holistic approach to care, addressing medical, behavioral, and social needs. You will work remotely, but your impact will be deeply personal, helping members achieve wellness and autonomy through education, support, and compassionate coordination.
Responsibilities
- Building meaningful relationships with patients and families, understanding their goals, barriers, and motivators.
- Creating personalized care plans that go beyond diagnoses, addressing social determinants and emotional well-being.
- Cooking care across providers and settings, ensuring safety, quality, and continuity.
- Educating and empowering members to make informed decisions and navigate the healthcare system confidently.
- Collaborating with an interdisciplinary team, sharing insights and advocating for whole-person health.
- Using data as a tool for insight—not the driver of care—to identify opportunities and measure impact while keeping the patient at the center.
- Maintaining compliance with regulatory standards while prioritizing compassion and connection.
Qualifications
- Active Illinois RN license
- Strong background in care management as well as complex and chronic condition management
- Skilled in motivational interviewing, patient education, and patient advocacy
- Ability to work independently in a remote environment while staying connected to the team
- Comfort with technology and multiple EMR systems
Specialty Qualifications
- Minimum of five (5) years of clinical experience across diverse healthcare settings
- Behavioral Health: Requires advanced knowledge in psychiatric conditions, crisis intervention, and therapeutic communication

Compensation
The listed compensation range is $80,000.00 to $83,000.00 paid bi-weekly per our standard payroll practices. Final base pay decisions are dependent upon a variety of factors including skill set, years of relevant experience, education, location, and licensure/certifications.

Remote Work Technical Requirements
Guidehealth is a fully remote company. We provide new employees with the necessary equipment to function in their role at no charge to the employee. Employees provide their own internet connection, capable of conducting video calls on camera and connecting to various internal and external systems.
